Confidentiality Agreement
Confidentiality and privileged communication remain rights of all clients according to state law.
Your identity is confidential; session content is confidential. I will not speak to anyone about you unless you are present and/or give signed consent.
However, certain situations can override your confidentiality. For example, if you are involved in court action, your file can be subpoenaed for court. If you make statements in session that a child or an elderly or disabled person has been abused or neglected, I am required by law to report that information to the appropriate authorities. If you make statements that indicate you intend to harm yourself or others, I may report that information to medical personnel, family, or law enforcement. In any case, you will be advised of the action immediately when it is taken.
USING YOUR INSURANCE FORFEITS YOUR CONFIDENTIALITY. Employers may be notified. You will be rated a higher risk for life insurance. Several employees at the insurance company may read your file. You will need a “Mental Disorder” diagnosis in order to use your benefits.
